La pollution de l’air pourrait avoir un impact sur le cancer du sein

D’après une étude, menée par des chercheurs français du Centre Léon Bérard au sein du département Prévention Cancer Environnement à Lyon, une exposition à long terme au BaP (benzo[a]pyrène) aurait un impact sur le cancer du sein pour les femmes.

Qu’est-ce que le benzo[a]pyrène (BaP) ?

Le benzo[a]pyrène est un hydrocarbure aromatique polycyclique (HAP) et fait partie de la famille des composés organiques volatils (COVs). Le BaP est un perturbateur endocrinien formé lors de la combustion incomplète de matières organiques (combustion mal maitrisée du bois, brûlage de végétaux à l’air libre, gaz d’échappement automobiles ou fumée de cigarette par exemple). C’est un polluant persistant préoccupant qui est classé dans le groupe 1 des agents cancérogènes pour l’homme, par le CIRC.

Quelle est sa source ? 

Les benzopyrènes sont produits par combustion incomplète de composés aromatiques :

  • Dans l’environnement des raffineries de pétrole, lors de la mise en œuvre du goudron, lors de la combustion de tous les combustibles fossiles. C’est un composant de la créosote (huiles extraites de goudrons (de bois ou de charbon ou d’une plante));
  • Dans les gaz d’échappement des moteurs Diesel en cas de combustion insuffisante.
  • Dans la fumée des cigarettes : elles produisent de 18 à 50 ng de benzopyrène;
  • Dans les vapeurs de goudrons;
  • Dans le chauffage au bois, qui est un émetteur d’hydrocarbures aromatiques polycycliques dans l’atmosphère;
  • Dans les brûlages de déchets verts. Ils sont aussi des sources de benzopyrènes à prendre en compte ; en milieu rural, la combustion du bois conduit à des niveaux élevés de B[a]P dans l’air extérieur;
  • Dans les barbecues, où le benzopyrène est produit par combustion incomplète des graisses animales au contact des flammes de charbon de bois.

Résultat de l’étude sur le lien entre pollution de l’air et cancer du sein

Cette étude a été réalisée grâce aux données de la cohorte nationale française E3N. Depuis 1990, un suivi d’environ 100 000 femmes, nées entre 1925 et 1950, est réalisé sur leur mode de vie (alimentation, activité physique, sédentarité, tabac, alcool, prise de médicaments, de traitements hormonaux, etc.), leur environnement, leurs histoire résidentielle (lieux où ils vivent et travaillent), et sur l’évolution de leur état de santé physique et mental.

Dans le cadre de l’étude effectuée par des chercheurs du centre Léon Bérard, ces derniers ont sélectionné les femmes de la cohorte qui avaient été diagnostiquées d’un cancer du sein entre 1990 et 2011 soit environ 5200 cas. 

Au cours de ce projet, l’équipe de recherche a évalué la relation entre l’exposition au BaP atmosphérique et le risque de cancer du sein dans l’ensemble de la population interrogée, en fonction du statut ménopausique des femmes, des sous-types moléculaires, du stade et du degré de différenciation du cancer du sein. 

Dans l’ensemble, l’exposition cumulée au BaP est significativement associée à un risque accru de cancer du sein. Ce risque dépend du statut ménopausique, du statut des récepteurs hormonaux et du degré de différenciation du cancer du sein. 

De manière générale, cette étude met en évidence le fait que la pollution de l’air peut être un facteur de risque de cancer du sein chez la femme. Actuellement le Centre international de lutte contre le cancer (CIRC), estime que seul le cancer du poumon peut être directement lié à la pollution de l’air (avec également des suspicions pour le cancer de la vessie).

Une prise de conscience de l’impact de la pollution de l’air

Selon Amina Amadou, chercheuse au Centre Léon Bérard et première auteure de l’étude, les personnes sont de plus en plus conscientes des effets sanitaires de la pollution atmosphérique. D’après elle « Si les effets sur la santé de manière générale sont bien connus (diminution de l’espérance de vie, augmentation de la mortalité), les liens avec les différents cancers, en particulier le cancer du sein ne sont pas établis. Les résultats de cette étude donnent des arguments pour limiter les émissions de polluants atmosphériques notamment les polluants ayant des effets de perturbateurs endocriniens (pouvant impacter l’équilibre hormonal des femmes). »

Ces résultats doivent être confirmés par d’autres études, mais les chercheurs estiment que ces derniers sont prometteurs car ils fournissent une nouvelle base pour limiter les émissions de polluants atmosphériques. Notamment les polluants ayant des effets de perturbateurs endocriniens, c’est à dire les polluants qui affectent l’équilibre hormonal des femmes.
